Dr. Mindy Forman obtained her bachelor of arts in Psychology from Michigan State University, a master of arts in Counseling Psychology from Wayne State University, and a Doctorate in Psychology from the Illinois School of Professional Psychology. She values the unique stories, strengths, and attributes each client brings to the therapeutic relationship, and tailors treatment to their individual needs.

We assess several referral questions including personality and emotional functioning, ADHD screening, IQ testing, and diagnostic clarification.
Evaluation begins with a thorough consultation, in order to design a specific battery to assess the referral question.
Testing sessions typically range from 6-10 hours, spread out over the course of one or two days.
One can expect a report in approximately two weeks after the final data has been collected, at which time a feedback session would be scheduled.

Do you offer tele-health?
Yes, most insurance companies are currently covering tele-health to support safety during the covid-19 pandemic. While the coverage continues to be extended, it is unknown how long the coverage without prior tele-health plans will persist. A self-pay option during the pandemic will always be available, should a client’s insurance not cover the service.
Is our work confidential?
Everything discussed in session is
confidential between client and therapist.
There are some limits to confidentiality
including concerns related to the safety of the client or others.
Some examples where others would need to be informed of risk
include cases of abuse, or threats of suicide or homicide.
How often will we meet?
We will meet weekly at the start of therapy, and may step down services gradually as you continue to make progress.
What will happen at our first meeting(s)?
The first meeting is a bit different than subsequent meetings. It is called a “clinical interview,” which is an opportunity for me to learn about you, and all of the factors that contribute to your current functioning. This first session involves more direct questions from your therapist; however, you are always in charge of what you choose to share.
What will happen in therapy sessions?
Typical sessions allow for the client to determine where and how the session begins. The primary function of the therapist will be to understand the client’s experience, and offer insights or suggestions that might help the client navigate the situation or their emotions more effectively.
What are your fees and accepted payment methods?
The fee for an intake session is $220, and $165 for 50 minute psychotherapy sessions. The practice accepts all major credit cards, HSA, FSA, and Zelle. You will be provided a bill and receipt for services, that can be available to you on the client portal through Simple Practice.
Do you accept insurance?
We currently accept Blue Cross Blue Shield PPO, and are in the process of credentialing with United Healthcare, Cigna, and Medicare.

Clients are responsible for verifying their insurance coverage. The practice will work together with clients to determine whether services are considered “in-network” or “out-of-network.” Out-of-network coverage may still cover a portion of services; however, this varies case by case.
Clients may also utilize a self-pay or sliding scale option if they do not plan to use medical insurance.
